The ongoing pharmaceutical marketing blitz promises that low-T treatment can make men feel more alert, energetic, mentally sharp, and sexually functional. An open-label study was performed to examine serum testosterone levels after treatment with IM TU using the approved dosing strategy for a total of nine injections (47). Testosterone levels were measured at baseline and at days 4, 7, 11, 14, 21, 28, 42, 56, and 70 after the third injection and 4, 7, 11, 14, 21, 42, and 70 after the fourth injection. A total of 130 hypogonadal males received treatment, but levels were available for 117. Overall, levels were similar after the third and fourth injections, with a mean Cmax of 813 ng/dL reached by day seven and a mean Cmin between 323 to 339 ng/dL by week 10 after each injection.

Based on serum testosterone levels, the dose can be increased in 25 mg increments up to 100 mg of testosterone daily. The 1.62% concentration is also available in a metered-dose pump and unit-dose packets (brand only). The metered-dose pump provides 20.25 mg of testosterone per actuation, while the unit-dose packets contain either 20.25 mg/1.25 g or 40.5 mg/2.5 g of testosterone (29).

Testosterone is a sex hormone that is made naturally in the body. Medicines that contain testosterone (also known as testosterone replacement therapy or TRT) work by adding more testosterone to a person’s body to help them achieve normal levels. Drug abuse is intentional non-therapeutic use of a drug, even once, for its rewarding psychological and physiological effects. Abuse and misuse of testosterone are seen in male and female adults and adolescents. Testosterone, often in combination with other anabolic androgenic steroids (AAS), and not obtained by prescription through a pharmacy, may be abused by athletes and bodybuilders. There have been reports of misuse by men taking higher doses of legally obtained testosterone than prescribed and continuing testosterone despite adverse events or against medical advice. There have been postmarketing reports of venous thromboembolic events, including deep vein thrombosis (DVT) and pulmonary embolism (PE), in patients using testosterone products, such as testosterone cypionate.

Exercise capacity in men with chronic heart failure increased after 12 weeks (60), whereas the cardiac index (cardiac output related to body surface) had already increased after 180 min. In men with coronary artery disease flow-mediated vasodilatation was observed after 12 weeks (61). Effects on sexual interest appear after 3 weeks plateauing at 6 weeks, with no further increments expected beyond. Changes in erections/ejaculations may require up to 6 months. Effects on quality of life manifest within 3–4 weeks, but maximum benefits take longer.

During the therapy, people will usually visit their doctor every few months for monitoring. However, testosterone did have associations with weight gain. It is also unclear what effect testosterone may have on other aspects of menopause, and the right dosage is also a subject of research. To accelerate development, doctors may consider a short-term treatment with low dose testosterone, particularly if the child is experiencing distress or bullying. Store at room temperature between 20 and 25 degrees C (68 and 77 degrees F).

Oral testosterone is easy to administer, but it must be taken with food. Although injectable therapy has flexible dosing, there is a risk of symptom fluctuations. Testosterone pellets have a long duration of action; however, they require an in-office procedure.

Morgentaler said more research is needed to understand why most men taking testosterone cypionate injections discontinue treatment within 1 year. Mean change in weight, BMI and BP was similar between the groups at 12 months. Testosterone injection may cause a decrease in the number of sperm (male reproductive cells) produced, especially if it is used at high doses.

Some products should be protected from light and should not be frozen. Some products should be kept in their original carton until you are ready to use them. Ask your pharmacist if you are unsure how to store your testosterone product.
